=== Anesthesia === Intravenous fentanyl is often used for anesthesia and as an analgesic. To induce anesthesia, it is given with a sedative like propofol or thiopental. To maintain anesthesia, inhaled anesthetics and additional fentanyl may be used. These are often given in 15–30 minute intervals throughout procedures such as endoscopy and surgeries and in emergency rooms. For pain relief after surgery, using fentanyl can decrease the amount of inhalational anesthetic needed for emergence from anesthesia. == Prognosis == The availability of treatments that modify the course of multiple sclerosis beginning in the 1990s, known as disease-modifying therapies (DMTs), has improved prognosis. These treatments can reduce relapses and slow progression, but there is no cure. Before the advent of these medications, it took around 15 years before relapsing-remitting MS progressed to secondary progressive MS (which has associated mobility issues). With these medications, the progression is slower to 40 years. The prognosis of MS depends on the subtype of the disease, and there is considerable individual variation in the progression of the disease. In relapsing MS, the most common subtype, a 2016 cohort study found that after a median of 16.8 years from onset, one in ten needed a walking aid, and almost two in ten transitioned to secondary progressive MS, a form characterized by more progressive decline. With treatments available in the 2020s, relapses can be eliminated or substantially reduced. However, "silent progression" of the disease still occurs. In addition to secondary progressive MS (SPMS), a small proportion of people with MS (10–15%) experience progressive decline from the onset, known as primary progressive MS (PPMS). Most treatments have been approved for use in relapsing MS; there are fewer treatments with lower efficacy for progressive forms of MS. The prognosis for progressive MS is worse, with faster accumulation of disability, though with considerable individual variation.

=== Dietary therapy === Initial attempts at dietary therapy in ALD involved restricting the intake of very-long chain fatty acids (VLCFA). Dietary intake is not the only source for VLCFA in the body, as they are also synthesized endogenously. This dietary restriction did not impact the levels of VLCFA in plasma and other body tissues. After the realization that endogenous synthesis was an important contribution to VLCFA in the body, efforts at dietary therapy shifted to inhibiting these synthetic pathways in the body. The parents of Lorenzo Odone, a boy with ALD, spearheaded efforts to develop a dietary treatment to slow the progression of the disease. They developed a mixture of unsaturated fatty acids (glycerol trioleate and glyceryl trierucate in a 4:1 ratio), known as Lorenzo's oil that inhibits elongation of saturated fatty acids in the body. Supplementation with Lorenzo's oil has been found to normalize the VLCFA concentrations in the body, although its effectiveness at treating the cerebral manifestations of the disease is still controversial and unproven. Trials with Lorenzo's oil have shown that it does not stop the neurological degradation in symptomatic patients, nor does it improve adrenal function, but asymptomatic patients, and speculatively AMN variants without cerebral involvement, as well as female carriers may benefit from early intake of oleic and erucic acids in addition to VLCFA restriction.
== Escalation == Initially positioned as a mission to stop narcotics traffic to the US, by mid-October 2025, Venezuelan opposition figures and independent analysts confirmed a shift in US objectives toward regime change, with Trump acknowledging the possibility of strikes within Venezuelan territory. On 15 October, Trump confirmed he had authorized the CIA to conduct lethal ground operations inside Venezuela and elsewhere around the Caribbean, and that military officials were drafting options for strikes on Venezuelan territory.
